Transfer Out Form

If you are transferring to another school within the U.S, please follow the steps below so that the WFU International Students & Scholars division (ISS) can initiate a transfer of your SEVIS record to your new school.

NOTE: Completing this form does NOT constitute notifying your academic advisor. Please be sure to discuss your transfer plans with your WFU academic advisor.







  • Please upload a pdf version of your acceptance letter
  • Name of the school you will be transferring into.
  • SEVIS school code of the school you will be transferring into (usually listed on their “transfer in” form, if applicable)
  • Name of your new school’s international advisor/DSO
  • This is the date you want your record transferred to the new school — usually the current semester or session completion date. IMPORTANT: We cannot “backdate” your transfer date; i.e., you must choose a current or future date.

    NOTE: If you are transferring from WFU after completing your program, the transfer out date can be anytime between the current program end date listed on your I-20 and your 60-day grace period.

    The hiatus between Wake Forest and your new school cannot be more than 5 months. Keep in mind the earlier the transfer date, the sooner the new school can gain access to your SEVIS record in order to issue your new I-20.
